MCaresNJ’s dedicated section for professional referrers is designed to streamline the process for caseworkers, social workers, discharge planners, and public health professionals seeking reliable, trauma-informed, and inclusive services for their clients. We accept referrals from hospitals, shelters, clinics, schools, justice programs, behavioral health organizations, and community nonprofits.

Professionals can submit referrals via our secure online form or fax/email. Referrals are acknowledged within 24 business hours. Case managers and referral sources are kept informed of intake status, and urgent cases are prioritized. This system is ideal for discharge planners, behavioral health providers, school counselors, and outreach workers.

MCaresNJ serves individuals of all ages, regardless of income, insurance, immigration status, gender identity, or housing status. Intake appointments include needs assessments, risk screenings, and benefit evaluations. Referring professionals will find our intake process particularly beneficial for clients requiring behavioral health triage, housing referrals, or access to HIV/STI care.

Our interdisciplinary intake team includes LSWs, certified peer specialists, bilingual case coordinators, and outreach navigators. They are trained in cultural competency, trauma-informed care, and community-based case management. A contact directory is provided for direct professional inquiries.

We actively partner with agencies to co-host trainings, attend multidisciplinary team (MDT) meetings, provide outreach staff at your site, and collaborate on shared service plans. Agencies are encouraged to reach out for lunch-and-learns, guest speakers, or mobile unit scheduling.
- Join Our Referral Network

Agencies referring multiple clients per month are invited to join our formal Referral Network. Benefits include priority intake access, quarterly service updates, co-branded programming opportunities, and input into pilot initiatives. Referrers will also be invited to professional development workshops and regional convenings.
