Our primary care services are designed for continuity of care and health equity, and serve as a vital referral resource for shelters, outpatient mental health centers, and recovery programs.
- Health Screenings

Full panel physical exams, including blood pressure, blood glucose, HIV/STI testing, and cancer screenings. These services are a key point of entry for uninsured or underinsured patients.
- Chronic Disease Management

Care coordination and medication management for clients with diabetes, hypertension, asthma, COPD, and Specialty Providers follow evidence-based protocols and regularly liaise with specialty providers.
- Women’s Health Services

Services include reproductive care, birth control counseling, pregnancy testing, Pap smears, STI testing, and menopause support. Referrals are often made from DV shelters, Title X clinics, and OB/GYN offices.
- Men’s Health Services

Annual wellness exams, prostate screening, STI testing, and cardiovascular risk reduction counseling. Ideal for community health workers and urgent care follow-ups.
- Pediatric Care

Wellness and sick visits, vaccinations, and growth/development screening for youth aged 0–18. School nurses and Head Start programs frequently refer children lacking consistent pediatric care.
