MCaresNJ’s case management programs address social determinants of health and are regularly used by housing providers, hospital social workers, and emergency shelters as supplemental or wraparound care.

Comprehensive, person-centered planning that includes needs assessments, care coordination, service navigation, and follow-up. Services reduce ER use and stabilize housing and health outcomes.
- Substance Use Support

Peer-led and clinician-supported recovery navigation for individuals with opioid, stimulant, or alcohol use disorders. Ideal for referrals post-detox or from MAT (Medication-Assisted Treatment) providers.
- Transportation Assistance

We offer door-to-door or subsidized transportation for healthcare, legal, and employment appointments, based on available funding or referrals through our partner agencies. These services are frequently accessed through referrals from medical providers, courts, and housing support teams.
- Basic Needs Support

Emergency provision of groceries, hygiene supplies, seasonal clothing, and survival gear. Serves as crisis relief for clients awaiting benefits or placement into services.
- Legal & Immigration Referrals

We refer clients to vetted legal partners for immigration advocacy, expungement services, housing rights, and name/gender change petitions. Highly requested by shelters, and DV agencies.
